Acceptable Use Policy - This acceptable use policy defines the information system security responsibilities and acceptable use rights for employees, volunteers, guests (including partners), vendors and contractors, students, medical students, residents, and medical professionals.
Information Security Policy - This information security policy is designed to support the Muskoka Algonquin Healthcare strategic plan and forms the basis of the Information Security Management System framework.
Internet Usage By Guests - The purpose of this policy is to define the appropriate uses of the Internet by patients, guests and visitors at Muskoka Algonquin Healthcare
Management of Personal Health Information Privacy Complaint or Breach - At Muskoka Algonquin Healthcare, all employees, physicians, students and volunteers are responsible for the protection of privacy and confidentiality of all written and electronic personal information (PI) and personal health information (PHI) following hospital security methods, ensuring prompt investigation and containment and promptly reporting any complaints or breaches of confidentiality and/or security. Awareness of any such incidents may come directly from the affected person(s) or from other parties.
Portable Devices and Media - At Muskoka Algonquin Healthcare, personal health information (PHI) is protected to ensure patient privacy. Under the Personal Health Information Protection Act (PHIPA), and pursuant to Orders HO-004, HO-007 and HO-008 from the Office of the Information and Privacy Commissioner of Ontario, health care providers are prohibited from storing unencrypted PHI on portable devices or media.
Privacy Policy - The purpose of this policy is to assert Muskoka Algonquin Healthcare's commitment to the protection of personal health information (PHI) from unauthorized collection, access, use, or disclosure, and protection of PHI from theft or loss. This policy addresses the appropriate collection, use and disclosure of PHI, the patient's right to limit access to his/her medical record, and the secure disposal of PHI when it is no longer required.
Processing Freedom of Information Requests - This policy will guide Muskoka Algonquin Healthcare in meeting its legal obligations and in maintaining an open, transparent and accountable culture while protecting the privacy of patients, clients and Staff.
Storage, Transport & Destruction of Confidential Information - At Muskoka Algonquin Healthcare, paper and electronic records containing personal health information and corporate confidential information are protected to ensure patient privacy and Hospital integrity.
Use of Social Media and Personal Devices -While Muskoka Algonquin Healthcare encourages its employees to enjoy and make good use of their on-duty and off-duty time, certain activities on the part of employees may become a concern if they have the effect of impairing the work of the employee; harassing, demeaning, or creating a hostile working environment for any employee; disrupting the smooth and orderly flow of work within the office; or harming the goodwill and reputation of the hospital among its patients/clients or in the community, at large.
