Your feedback is anonymous. The survey is administered by SOM, an independent research firm, to protect your privacy. Your individual responses will not be shared with either your manager or MAHC leadership.

As a thank you, everyone who completes the survey will be offered a $5 Tim Hortons digital gift card. You'll have the option to provide your MAHC email address at the end of the survey.
