Diabetes Education Program

Expert education and support are essential to help prevent and manage diabetes.  Our diabetes education team consists of Registered Nurses and Registered Diabetes who hold advanced credentials as Certified Diabetes Educators.  Additional expertise is provided by a Certified Insulin Pump Trainer and visiting physician specialists, such as an endocrinologist, providing care to adults living with diabetes.  Together, this multi-disciplinary team works to help our patients learn as much as possible about living with diabetes from our Diabetes Education Programs in Bracebridge and Huntsville. 

Services are also provided to residents of East Parry Sound at the Almaguin Highlands Health Centre in Burk's Falls*.  

Appointments are required through the referral process.

Services

Individual appointments for adults:

  • new to diabetes
  • starting insulin therapy
  • diabetes education through pregnancy
  • monthly multi-disciplinary clinic in Bracebridge by a visiting Endocrinologist

Virtual diabetes education and training are also provided for health care professionals and community service partners.

Preparing for Your Appointment

How do I get referred to the program?

You can either be referred by your primary health care provider, you can self-refer by calling our Bracebridge site at 705-645-4404 ext. 3600, or Huntsville site at 705-789-2311 ext. 2312.

What should I bring to my appointment?

Please bring:

  • your health card,
  • a list of your medications, and
  • your blood glucose meter, or record of recent blood glucose readings.

What can I expect during my first appointment?

If you’re attending a class, you’ll receive education to manage Type 2 Diabetes.

If you arrive for a one-on-one counselling, you’ll be provided with a blood glucose meter if you require one, and you’ll receive personalized education and resources to bring your blood glucose to target.

Referrals

You can make an appointment with our Diabetes Education Program directly, or your doctor or health care professional can send a referral.  When we receive the referral, we will call you to make an appointment.
