Palliative Care is provided at both the Huntsville District Memorial Hospital and South Muskoka Memorial Hospital sites. Our focus is to give patients comfort and to support independence and participation in leisure and self-care activities. Palliative Care includes psychosocial, physical and practical support.
Muskoka Algonquin Healthcare appreciates the support provided by Hospice Muskoka in Bracebridge and Hospice Huntsville/Algonquin Grace, which through professionally-trained volunteers provide compassionate care so that individuals may live out their lives with dignity and comfort either at home or in a health care setting.
South Muskoka
At the South Muskoka Memorial Hospital Site, palliative care is provided by both the North Simcoe Muskoka Hospice and Palliative Care Network (NSMHPCN) and by Hospice Muskoka. NSMHPCN palliative nurse consultants are available for consultation and are specialists with more than 25 years of palliative experience between them in secondary level specialty care, practicing the palliative approach to care. As leaders in disease-modifying care, pain and symptom management, giving attention to highest quality of life possible and compassionate end-of-life care, the NSMHPCN nurse consultants have specialty training in palliative care through education and clinical practice including Canadian Nurses Association (CNA) Hospice & Palliative certification, Learning Essential Approaches to Palliative Care (LEAP) and Comprehensive Advanced Palliative Care Education (CAPCE) palliative leadership education. The presence of the NSMHPCN Nurse Consultants at the SMMH Site broaden access to palliative care for the South Muskoka community.
Hospice Muskoka provides services at home, in hospital, in seniors' residences and nursing homes in the Towns of Bracebridge and Gravenhurst and in the Township of Muskoka Lakes. Hospice Muskoka has set up and furnished the Palliative Care Family Room at the SMMH Site and provides a visiting volunteer program for palliative patients. Huntsville
Hospice Huntsville provides care and support to individuals in the Huntsville area who are facing end-of-life situations. They provide support to families and caregivers in the setting of their choice, including Algonquin Grace, a five-bed residential hospice serving the community, residents and their families in Huntsville.
