211 Community Connection

211 is a three-digit phone number like 411 and 911. 211 helps people find services in their community using a live answer service 24 hours a day, 7 days a week.

211 is free to call and interpreters are available in more than 170 languages.  TTY service is available at 1-888-435-6086.

In Ontario, 211 is funded by the Ontario Ministry of Community and Social Services and many municipalities and United Ways.

Common Point Mental Health & Addictions
Common Point is a web-based tool to help find mental health and addiction services in North Simcoe Muskoka.
Family Health Teams

Family Health Teams are health care organizations that include a team of family physicians, nurse practitioners, registered nurses, social workers, dietitians, and other professionals who work together to provide health care for their community. There are a number of Family Health Teams in the Muskoka area:

Find A Doctor

Family doctors or Nurse Practitioners provide primary care in the community and operate independently of the hospital. 

The provincial government operates Health Care Connect, a service helping Ontarians without a family health care provider to find one. People without a family health care provider are referred to a family doctor or a nurse practitioner who is accepting new patients in their community.  You can sign up for Health Care Connect if you do not already have a family doctor.  Read more information from the Ministry of Health about the Health Care Connect program.

Health Hubs

Community Health Hubs have been developed to enhance access to primary care and other community programs and services in rural and remote communities in the District of Muskoka.  Health Hubs are funded by the Ministry of Health and Long-Term Care and are in the community as follows:

Holiday Hours Website

The Holiday Hours website validates holiday hours for health care facilities nationally to help patients find care during holidays throughout the year and long weekends.

Home and Community Care Support Services

Anyone can call to find out about home and community care services provided through Home and Community Care Support Services. If you would like to access care, or if you are currently receiving care and have questions, call 1-888-721-2222 or 310-2222 (no area code required). A responsive team of staff are available to answer your questions and help you access care at home in your community.

Hospice

Hospice provides care and support to individuals who are facing palliative and end-of-life situations.

Hospice Huntsville provides care and support to individuals in the Huntsville area.

Hospice Muskoka provides care and support to individuals in of South and West Muskoka.

The iamsick.ca website is an up-to-date directory of all health care facilities nationally, including family doctors accepting patients, confirmed holiday hours, languages spoken by health care professionals, and detailed information about services offered.
Midwives of Muskoka
Midwives are regulated health care professionals who provide primary care to women throughout pregnancy, labour and birth, and to mother and baby for six weeks following the birth.  Midwives of Muskoka has offered midwifery care across the Muskoka region since 2005.  Midwives are community-based, and have hospital privileges at both Muskoka Algonquin Healthcare sites. Midwifery services are funded by the Ministry of Health and Long-Term Care. 
Muskoka and Area Ontario Health Team
The Muskoka and Area Ontario Health Team is one of the first 24 teams in the province to implement a new model of organizing and delivering health care that better connects patients and providers in the communities to improve outcomes. Through an Ontario Health Team, patients will experience easier transitions from one provider to another. By integrating and organizing to deliver services locally under a collaborative partnership model, the Muskoka and Area Ontario Health Team has a vision to build a health care system that is uniquely Muskoka.
Muskoka Health Link

Health Links are an initiative of the Government of Ontario to support and encourage greater collaboration between existing local health care providers, including family care providers, specialists, hospitals, long-term care, home care and other community supports. With improved coordination and information sharing, patients will receive faster care, will spend less time waiting for services, and will be supported by a team of health care providers at all levels of the health care system.

The Muskoka Community Health Link Steering Committee was formed in the winter of 2013 and is a partnership between the District Municipality of Muskoka, the Algonquin Family Health Team and Cottage Country Family Health Team, the North Muskoka Nurse Practitioner-Led Clinic, the North Simcoe Muskoka Community Care Access Centre, Muskoka Algonquin Healthcare, Canadian Mental Health Association, Muskoka-Parry Sound Branch, and other community agencies.

Muskoka's Community Health Link is focused on:

  • increasing access to primary care through Muskoka Community Health Hubs;
  • improving system navigation through co-ordinated care planning;
  • reducing Alternative Level of Care needs through community capacity building.

The myflushot.ca website provides flu shot options across Canada, as well as public health influenza resources.
NSM Healthline

NSM Healthline is an access point for health information in North Simcoe Muskoka, providing a wide inventory of health and related social services and centralized health resources, including specialized portals to services, health information, job opportunities and regional events for the areas surrounding Barrie, Collingwood, Midland, Muskoka and Orillia.

NSM Healthline is available by phone at 310-2222 or 1-888-435-6086 (TTY) where your call will be answered live, daily until 8:30 p.m. or by visiting www.nsmhealthline.ca.

Nurse Practitioner Clinics

Nurse Practitioner-Led Clinics are a new primary health care delivery model in which Nurse Practitioners are the lead providers of primary health care.  Through a collaborative practice approach which includes Registered Nurses, Registered Practical Nurses, collaborating family physicians, and other health care professionals, nurse practitioners provide comprehensive, accessible, and coordinated family health care services to populations who do not have access to a primary care provider (i.e. unattached patients).

Rural Nursing Stations are also located in Britt, Argyle, Moose Deer Point, Pointe au Baril, Rosseau and Whitestone and are operated by the West Parry Sound Health Centre.

Ontario Caregiver Organization

The Ontario Caregiver Organization (OCO) exists to support Ontario’s 3.3 million caregivers; ordinary people who provide physical and emotional support to a family member, partner, friend or neighbour.  Working collaboratively with caregivers, healthcare providers and other organizations, OCO draws on the variety of work that is currently being done to ease caregiver burn-out and improve the caregiving experience.

Ontario Health is an agency created by the Government of Ontario to connect, coordinate and modernize Ontario’s health care system, working with partners, providers and patients to make the health system more efficient so everyone in Ontario has an opportunity for better health and wellbeing.

Simcoe Muskoka District Health Unit
The Simcoe Muskoka District Health Unit is a public health agency that offers a wide range of programs and public health services focused on promoting and protecting health, and preventing disease, disability and injury.
Telehealth Ontario

Telehealth Ontario is a free, confidential telephone service at 1-866-797-0000 or 1-866-797-0007 (TTY) providing you with access to experienced health advice or general health information from a Registered Nurse 24 hours a day, seven days a week.

The service provides quick and easy access to a qualified health professional who can assess your symptoms and help you decide your best first step. The service can help you decide whether to care for yourself, make an appointment with your doctor, go to a clinic, contact a community service or go to a hospital emergency room.

Remember, a call to Telehealth Ontario does not replace 9-1-1. Always call 9-1-1 for emergency situations.

Trillium Gift of Life Network

Trillium Gift of Life Network (TGLN) is the Government of Ontario agency responsible for planning, promoting, coordinating and supporting organ and tissue donation and transplantation across the province and for continually improving the system so that more lives can be saved.

In 2013, Muskoka Algonquin Healthcare became a designated hospital under the TGLN Act. This designation means that MAHC participates in routine notification, which is the rate at which hospitals notify TGLN when there may be potential for organ and/or tissue donation. By implementing a formal program, we are establishing a culture of donation and raising awareness of the organ and tissue donation need across the province.

Today, in Ontario, over 1,500 people are waiting for a lifesaving organ transplant. For many, this is their only treatment option, and every three days someone will die because there aren't enough organs to meet the need.

By registering to become a donor, you have the power to save or change someone's life. One organ donor can save up to eight lives and enhance as many as 75 more through the gift of tissue.
